Eli Lilly Is Setting Up for Another Leg Higher Before Earnings

Signal: BUY | Score: 65.46/100

Lilly just got three FDA approvals in two weeks, its PBM coverage headaches are largely resolved, and the stock is sitting about 8% below its 52-week high with earnings 22 days away. That combination doesn't come around often for a mega-cap pharma name this clean.

The Setup

LLY is trading around $1,187, off its $1,292 peak but well above the $783 low it printed earlier this year. The RSI at 57 tells you this isn't overbought - there's still room to run before momentum gets stretched. Momentum score is 70, sentiment is 72, and the stock is in a healthy uptrend with no signs of distribution. This looks like a trend continuation play, not a speculative bet. The stock is coiling below resistance and has a clear runway back toward the high.

The Catalyst

Three things are working right now, and a fourth is coming.

On September 24, the FDA approved Onswik, a once-weekly basal insulin that extends Lilly's already dominant diabetes franchise. Then on October 1, CVS Caremark expanded Zepbound coverage, joining the other major PBMs in restoring access to Lilly's weight loss lineup. That PBM overhang was the single biggest cloud over the stock for most of 2025 and early 2026 - it's now clearing. And on October 2, the FDA expanded Jaypirca into first-line CLL/SLL, adding oncology diversification that the market hasn't fully priced in yet.

The big one is October 29 - Q3 earnings. JPMorgan has a $1,500 target, RBC reiterated Buy on October 2, and the consensus is that Mounjaro and Zepbound demand is accelerating into the back half of the year. If Lilly raises guidance, this stock goes straight back to new highs.

Bull Case

  • Analyst consensus target sits at $1,329, representing roughly 12% upside from here with JPMorgan's $1,500 target implying 26% if the earnings print is strong
  • Expanded PBM coverage is a direct revenue driver for Zepbound and Mounjaro - Q3 results will be the first full quarter reflecting that tailwind
  • Forward PE of 25x is reasonable for a company growing revenue at 48% year-over-year with a 33.5% profit margin

Bear Case

  • At 31x price-to-book and 13x sales, there's no margin for error - a guidance miss or supply concern on October 29 would hit hard
  • Debt-to-equity at 162 is elevated, which matters more if the macro environment shifts toward higher rates

The Trade

  • Entry: $1,185 to $1,195
  • Target: $1,329 near-term, $1,500 on a strong earnings beat
  • Stop Loss: $1,095 (just below the August consolidation range)
  • Risk/Reward: Approximately 2.5 to 1 on the base target
  • Timeframe: 4 to 6 weeks into earnings and beyond
